It teaches us a lot about human nature and how to improve as individuals, and also how to ensure all the things we do with others are done as reasonably well as we can. I also just think it’s very cool to understand the details about how groups of us ordinary people can collectively do analyzing things.

The key error seems to be that somehow, perhaps because Boeing was working with a site-visit team from Spirit to correct another problem from the Spirit factory at Boeing instead of back “home” to save time, the paperwork wasn’t submitted for documenting the removal of the locking bolts, a fatal error when for reasons not yet identified the bolts weren’t put back on in the normal course of business and that wasn’t caught in inspection.
This error was evident in the internal chat log — that is, the work was noted in unstructured chat but the paperwork was missing — so this is exactly the sort of thing AI monitoring could catch, even right now.
