#AIR-CLINIC ROCKS! It’s good to be back with another interesting episode of Online Ward rounds, a special programme for medics on the blockchain. It is a part of @Air-Clinic's sub-project,
SMSI (steem medics support initiative).

Case 1

A 23-year –old woman develops an acute pain in her right chest radiating to her right shoulder, there is an associated fever and vomiting with a mild yellowing of her skin.

  • What is your diagnosis? (And what is the basis for making your diagnosis?)
  • What are your DDx?
  • How would you work up this patient?
  • How would you manage this patient?

Case 2
A 16- year-old mother brings her baby for immunization. The Nurse notices the baby has multiple bruises along both arms and legs. The baby is crying excessively. The house officer also notices that the baby has multiple fractures with a blue sclera.

  • What is your diagnosis? And what is the basis for making the diagnosis?
  • What are your DDx
  • How would you work up the patient?
  • What are your management modalities for this case?

My name is Evelyn Foster, from Nigeria and a Pharmacist by profession. I also have a Masters degree in Public Health

Why you love @air-clinic

I love @air-clinic because it's a place where you can get so much information about health-related issues. It's like a knowledge bank. Also, because it runs free consultation for clients without charging a dime. In fact, it's really solving the health problems of many people.

High and low point on steemit

Well, my high points on steemit was when I joined until recently. Then, the price of Steem and SBD were good so payouts were good. Another high point is the networks and connections I have built on steemit. They're very important to me.

How do you balance your steemit life and real life

Well, nowadays, I really try to draw a balance between steemit and my everyday life. Before I must say I spent so much time on steemit. However, I now know how to spend the appropriate time on steemit. I try to make 1 post a day and 10 comments every day.

Motivation to study your career.

I was motivated to study Pharmacy and Public Health because I wanted a platform through which to reach out and help others live healthier lives.

Challenges/survival story in your specialty.

I have not really had challenges in my specialty so far.

I think it is a liver abscess, due to fever, nausea and jaundice.
differential diagnosis with vesicular lithiasis, with cholecystitis and cholestasis, with probable migration of the stone to the pancreas. It can also be a collection on the right lung base
You have to practice abdominal ultrasound. Chest x-ray and blood tests complete hematology, with total and fractionated bilirubin, phosphatases, transaminases. To decide or not surgical resolution.

Case 2
It is an imperfect osteogenesis, based on multiple fractures and blue scleras. The diagnosis is clinical and radiological, in addition to the molecular study of collagen in a skin biopsy.
Is mandatory in cases like this, with multiple fractures, rule out child abuse

The current treatment is bisphosphonates, which have the ability to bind to bone and stimulate osteoblasts, thus inhibiting the activity of osteoclasts, pain treatment, surgeries for some fractures and continuous medical checks.

Case 2
My DX is osteogenesis imperfecta.

Basis H/o multiple fractures and blue sclera.

Ddx- OI, Battered baby syndrome, rickets , osteomalacia.

Work up-

  • Medical imaging (x ray) will show multiple fractures.OI cortical bone shows increased porosity, canal diameter, and connectivity in micro-computed tomography

  • DNA or collagen testing is confirmation.

  • Skin biopsy can be performed to determine the structure and quantity of type I collagen.

Treatment

  • Treatment includes care of broken bones, pain medication, physical therapy, braces or wheelchairs, and surgery. A type of surgery that puts metal rods through long bones may be done to strengthen them.

and i also noticed you have a penchant for the second cases..(winks)

Basically i usually see cases after an hour or so and i expect others to have started working on first case. Morever , i attempt only one case so that max people get chance to participate and enlighten us.

For case 1
My diagnosis is cholelithiasis + acute chole-cystitis(angitis) with differentials of Acute pancreatitis, Acute hepatitis,

Reason.. Acute pain, fever, right sided chest pain radiating to the right shoulder, yellowness of skin.

Investments will include CBC, LET, urinalysis, Abd USS, CT, ERCP

Treatment is with analgesics, hydration, antibiotics, antiemetic, stone dissolution (oral bile salt therapy, extracorporal Shockwave lithotripsy) or surgery( cholecystectomy).

@air-clinic, i think she already joined a long time ago, its just that she has not been coming around. I will try and work on her to be more frequent on the server

Case 1. It could be cholangitis due to lithiasis in the bile ducts or gallbladder. The pain in this area is usually referred to the shoulder on the same side. Abdominal ultrasound and laboratory should be performed to corroborate diagnosis, start antibiotics and analgesics and then schedule surgery.
Case 2.Definitely it is about osteogenesis imperfect, the finding of multiple fractures in childhood without apparent trauma with blue scleras are diagnostic criteria. These cases should be treated with bisphosphonate cycles to avoid fractures.
