CoinDash.io, within the first 11 minutes into their ICO, their website was compromised. The hacker replaced the donation address with his personal Ethereum address, with donors still sending coins to his address.
Some users are speculating that those behind the ICO orchestrated this themselves, although there is no evidence behind that idea at the moment.
