Knee Osteoarthritis is the chronic degenerative disease that affects your articular cartilage of synovial joints, leads to the overgrowth or bone spur formation at the margins of knee joint. Joint pain, stiffness in early morning are the common symptoms shown to the patient.

Osteoarthritis can also occur in other joints like hands and hip joint. But mainly affects knee joint in older individuals. It can occur due to genetic factors but also due to sedentary lifestyle, malnutrition, osteoporosis and many more. You can develop knee Osteoarthritis if you have family history of knee OA or OA at any other joint.

Clinical Signs and Symptoms

There are following sign and symptoms of knee OA associated with pain that can lead to hypomotility or hyper mobility of knee joint.

  • There is knee pain along with external swelling in some conditions not in all patients.
  • Morning stiffness, crepitations and limited ROM due to joint restrictions or thickening of articular cartilage.
  • Weakness of knee flexors and extensors, abnormal alignment of knee joint like bowing of knee.

Grades of Knee Osteoarthritis

There are 5 grades of knee OA that are on the basis of symptoms and that are severe symptoms at grade 5.
Grade 0.

  • There is not X ray findings on grade 0.

Grade 1

  • Doubtful space reduction at knee joint.

Grade 2

  • Significant narrowing of space in knee joint and osteophyte formation starts ( extra bone growth).

Grade 3

  • Moderate reduction of knee joint space and multiple osteophyte formation.

Grade 4

  • Severe and last grade of narrowing of knee joint space and joint to joint contact occur.

Assessment

  • You can assess patient of knee by taking past history related to disease.
  • Physical examination of patient assess swelling, tenderness around knee joint.
  • Thorough examination like investigations ( X ray, MRI) and special tests like McMurray tests, Quadriceps strength test etc.

Physical Therapy Intervention for Knee OA

Physical therapy intervention for knee OA are from months to years to regain normal ranges and subside pain with movement or at rest. Here are the few exercises videos that you have to perform to treat knee OA patients. Treatment protocol vary from patient to patient. But the main thing is you have to evaluate the exact cause of knee OA either it's genetics or other diseases related.

Heel Slides exercise

This the exercise to strengthen muscles around your knee and prepare the patient for walk. It also perform to decrease friction between heel and ground to walk smoothly.

William Flexion ( Knee to Chest exercise)

This exercise is perform to improve knee flexion, also patient can self stretch hamstring muscles to increase ROM.

Knee Isometrics ( Knee Flexion )

Knee isometrics are performed to improve knee and hip muscles strengthening, to increase ROM of knee joint. You have to press the towel downward to use quadriceps muscles and improve muscle strength.

Knee Isometrics ( Hip abduction & adduction)

Knee isometrics are performed to improve knee and hip muscles strengthening, to increase ROM of knee joint. You have to press the pillow between you legs to improve hip adduction and for hip abduction you have to use thera band or provide resistance with your hands to prevent more outward moving of knees. These exercises will strengthen you hip & knee abductors and adductors.

Calf & Hamstring Stretch

Calf & hamstring stretch is to improve the shortened muscles that is because of pain and immobility. You have to perform in supine, take a bend place below your foot, dorsiflexion on ankle joint and then calf & hamstring both muscle groups stretch at the same time.

5 to 10 minutes heat up before exercise is necessary as muscles relax after heating then you can perform exercise easily. Perform 5, 5 repetitions on first week then increase accordingly. These are the exercises for beginners as other exercises can by performed by professionals and experienced physiotherapist or gym trainer.

Homework Task
  • What's knee OA? Write in your own words after getting knowledge from the lesson post.
  • How would you diagnose a knee OA? Any clinical investigation or assessment tests?
  • Try to practice at least 4 exercises that you have learned from the lesson. Share images, gifs or videos while practising.

Do I need to perform these excercises on someone else or can I perform them myself because I think it'll be more beneficial if I perform them myself.

Yes, you can perform it on yourself fir better results. But if you can't perform then you can perform on someone else. I hope you understand.
