I'm sure that everyone out there knows one of these people. I don't know how they manage to pull it off but they pay zero attention to what they eat or drink, they rarely if ever exercise, maybe they even smoke as well, yet somehow they manage to be skinny as a rail with zero effort.

There are a few people in my life that are like this and they are all men. For some reason, this phenomenon doesn't seem to happen to any of the women that I know although I am sure they are out there somewhere. These people are probably even moreso envied than then men with this blessing / curse that they have.
Dave is one of my friends that is like this. He is constantly asking me to go to the bar. When he is there he drinks full bodied lagers and never pays any attention to how many carbs or calories are in them. He orders fried bar snacks while he is there and the few times I have been to his house I don't think a piece of fruit has ever been in his cupboards.
He pays exactly zero attention to his health yet he is not overweight at all. A lot of people look at this situation and think about how lucky Dave must be to be like this when the rest of us constantly struggle with our weight but I found something out about Dave the other day that kind of surprised me. He actually hates the fact that he can't seem to gain weight and it concerns him that there might be something wrong with him. I think this is a very rational fear because let's think about all of our portly relatives that when they were dying, they always lose a ton of weight first.
Dave is too young for this to be a real fear of his, but he actually tells me that he is afraid to go to the doctor because he is fearful of what the bad news could end up being. I suppose this is a reasonable fear because if it is something terribly wrong with him that is incurable, I would rather not know about it as well. Dave also says that he has tried working out, but never gains any noticeable muscle mass or strength. He is skinny as can be and doesn't really enjoy being tiny despite the obvious advantages of being able to drink and eat whatever he wants with what appears to be zero consequences.
According to what i can find online, the only favorable biological reasoning behind this is being blessed with a magically fast metabolism and a lot of lean tissue fibers that naturally require more calories in order to function. Dave is in his early 30's, so we could see things change a lot for him in the coming years since this is right around the time that the metabolism starts to slow down for most men. In the meantime, provided there isn't something terribly wrong with Dave and the others that I know with this affliction, I am extremely jealous.
I do encourage Dave to do cardio though, because at the end of all of this, exercise should be about maintaining some level of health, not just getting a "perfect" body size. When Dave's metabolism slows down, he already has all the bad habits, but none of the good ones to balance them out.
