When Dan Ellsberg interviewed military and executive civilians involved in the control of our nuclear arsenal he found that once a strike signal was initiated there was no provision for interrupting it by pause or reversal.

He interviewed and asked decision makers in the command system what would they do under various hypothetical conditions, and it revealed the world's extreme vulnerability to elementary error in unleashing a nuclear holocaust.

Indeed this was born out in the Kennedy-Russia (Khrushchev) confrontation in the CUBAN missile crisis.

Above, the topic is National Security not cold war mutual assured destruction, but the issues are the same even if the instantaneous risks are less prominently vulnerable.
