It's a crackup isn't it. But I have to admit I didn't do that one myself - it came through on my GAB feed and I copied and pasted it.
I do sometimes do edits like that - in Photoshop, essentially the Time cover would be the background layer and the Joe the pervert layer would be on top, and carefully positioned and erased around the edges until it looked right. All this can also be done in the free GIMP program, and I imagine there are apps that would do it too.
