Hello friends
I hope you are good I am here to take part in learning challenge that is really good for us as it give me chance to learn about the things which are needful at home under different circumstances Steemit Learning Challenge-S21W1: Physical Therapy Intervention: Frozen Shoulder, thank you @ashkhan for given us this opportunity.
1: What's frozen shoulder? Write in your own words after getting knowledge from the lesson post.
First of all I will describe the term what is frozen shoulder another name for this disease is called adhesive capsulitis it means that due to inflammation in the capsule leads to its adhesion which result in automatically movement of the shoulder is limited it become hard and stiff and the patient complaining of pain in the joint.
Why this is happening as I mentioned the connective tissue which is around the shoulder joint is swollen due to inflammation which leads to mobility limitations.
The patient is complaining that the hard shoulder joint is difficult to move there are different degrees of movement of our shoulder The most important is the upward rotation of the shoulder. The patient complains that while I am sleeping I have pain during midnight and I can't move my shoulder.
Pathogenesis
For example the patient who are very active or do not do any workout and it is most commonly seen in the women after 50s. The patient of diabetes or any autoimmune disease for example SLE. and most commonly due to any RTA or trauma.
Staging
Firstly pre-freezing stage along with that external rotation is limited and patient complaint of midnight pain. Then shoulder is undergone second stage freezing which leads to reduced movements and chief complaints is pain.
Frozen shoulder means now patient is coming with unable to move shoulder and it seems so hard.
Recovery phase which is called thawing means after inflammation settled movements come back.
There are 3 phases of the frozen shoulder.
Acute
In this phase patient complain of pain lead it to most important thing limitations of the abduction and external rotation.
Subacute
Patient feels hard and stiffness in the shoulder and it's internal rotation is not affected.
Chronic
In the chronic phase deltoid muscle also affected and patient unable to do over the head movement if you ask patient to touch your back he will not.
Treatment
First such patients visit physiotherapist for physiotherapy and if they are complaining of pain painkillers are given.
For stiff shoulder need to do certain exercise. And if with medicine can't cure surgery would be recommended.
2: How would you diagnose a frozen shoulder? Any clinical investigation or assessment tests?
For diagnosis you must have good experience and clinic assessment.
Properly take all the medical history of any previous RTA any autoimmune disease must ask how long he/she suffering from the disease to differentiate whether which phase.Which type of movement limitations and where is the main pain.
Exmaination
Inspection
Must inspect properly patient posture with shoulder.
Palpation
Palpate for pain hot or the swollen or not.
Range of motion
There are different categories of the movements as I mentioned in some phases abduction and internal or external rotation effected.
Some common test can be performed
Speed test ask patient to do abduction and internal rotation to see which stages he is suffering from.
important investigations
First main simple thing is x-ray to see whether any bone is fractured or not.
Second if X-ray is clear still pain go for ultrasound to see any soft tissue injury.
And if on the ultrasound soft tissue injury is seen must go for MRI for the confirmatory diagnosis of the adhesive capsulitis.
Another exceptional test which is called EMG to see any muscle injury.
What is the criteria to confirm diagnosis
ROM is almost less then 40%
Swollen red and hard shoulder joint
Patte's test positive
Along with that investigation confirm.
3: Try to practice at least 3 exercises that you have learned from the lesson. Share images, gifs or videos while practising.
Finger ladder
Wand shoulder flexion
Cross body stretch
Finger ladder
Shoulder flexion will increase and increase strength of shoulder.
Wand shoulder flexion
Also improves shoulder flexion one hand assist other Also improves strength.
Cross body stretch
For muscle strengthening and Also improves its mobility
4:Share your review after performing these exercises either on healthy individual or patient.
Well I have practice these exercise on myself, I feel much relaxed and as I have issues of shoulder stiffness due to post caesarian. Now I am feeling good after performing of these exercises.
Thank you dear @ashkhan for this wonderful opportunity.
Its all about my today's post.
I would like to invite @iqrarana786, @m-fdo and @jannat12 to take part in chllenge.
Thank you for understanding the lesson and sharing your assignment; I hope that you will enjoy this week's lesson and try to implement it in your life if you see any such case.
Observations
Task 1 (3/3)
You have shared a great knowledge about frozen shoulder, it's phases, types , stages, pathogenesis and treatment. I am glad that you write about the phases of frozen shoulder that has been skipped by all participants. I appreciate your effort.
Task 2 (2.5/3)
In the second question, you tell us about the physical examination like inspection, palpation, investigations, special tests. But the test you have shared is less common the more common tests are speed test, yergason test. You answer this question well. Good.
Task 3 (3.7/4)
You try the wand shoulder flexion, finger ladder, and cross body stretch in a good way. But while performing finger ladder your arm should be straight and keep a little more distance from wall. Remember always apply heating pad before performing these exercises. I appreciate your efforts.
Overall you made a good try to answer all the questions. I appreciate your efforts. But next time try to avoid the above written suggestions. Keep learning and try to implement your knowledge to the people suffering from any shoulder pathology or you suffer from such condition. Thank you.
