The Thoracic Outlet Syndrome (TOS) is group of conditions in which there is compression or irritation in the nerve or nerve vessels which are passing through the thoracic outlet.

Now this compressions on nerve or blood vessel is responsible for shoulder and neck region pain. Moreover it also causes the numbness in the fingers.

I will discuss what are the symptoms of thoracic outlet syndrome, the first sign is the pain especially the the neck, shoulder and the arm region.

Secondly it extended with time and may become the reason of the numbness in the hands and in the fingers too.

There is sensation of tingling in the hands and also I the fingers passively. Moreover there is fatigue on the hands and also in the fingers.

In case of severe it may be the reason of discoloured and swollen hands.

Now what are the causes?

The involvement of certain anatomical abnormalties are become the reason.

The injuries and repetitive motion of shoulder and neck is also another major reason.

Hence correct body posture is important if we are not in correct posture it might be associated for this syndrome.

Few common types of syndrome are follows.

The Neurogenic TOS, in this case there is compression of nerve which becomes the reason of pain and weakness in hands and arms.

The Venous TOS: in this case the compression of the veins is becoming the reason of swelling and also discoloured in hands and arms.

The Arterial TOS involved the compression of the arteries and it's the reason of the , coldness or weakness in hands and arms.

The Stages of TOS

Acute Stage it's duration ranges less than 14 days and in this car there is pain, numbness and tingling in hands or arms.

The time period of Subacute Stage may ranges from two to six weeks and it associated pain, numbness, tingling sensation.

The Chronic Stage may started after 6 weeks and has chronic persistent pain and numbness in the hands and arms.

The Advanced Stagehas long term with associated severe pain and weakness in arm and hands.

The diagnosis firstly done by physical examination by checking numbness and pain. Furthermore the x-ray, MRI and CT scan are done.

Diagnosis through physiotherapy

These special Tests are as follows.

The first test is called Roos Test it's used for diagnosis of TOS also involved to check external rotation of the arm.

The Wright Test is particularly important while assesment of the hyperabduction of the arm.

The used of Adson Test is performing to checking the palpation that is of radial pulse.

A test used to diagnose TOS, involving palpation of the radial pulse during arm elevation.

The used of Tinel Sign: for the diagnose of nerve compression, it's involved tapping over the nerve.

You have shared a great knowledge about thoracic outlet syndrome, its symptoms, causes, types but didn't add its other names, separate the vascular and neurogenic symptoms of TOS. I appreciate your effort.

In the second question, you tell us about how you have to assess a patient by doing physical examination, history taking, special tests and investigations. It would be better to add special tests and also explanation of special tests. And it would be better to perform them on a patient or healthy individual as I have share the pictures of special tests patient positions in lesson post. Good.

Task 3 (3.5/4)
You try the shoulder shrugs, external rotation and extension. You did the shoulder shrugs, external rotation correctly. But it would be better keep mobile on wide angle as extension exercise is not clearly visible. Always remember to apply heat pack in chronic condition and ice pack in acute condition before performing exercises to relax the muscles and reduce stiffness. I appreciate your efforts.
