As details emerge, it's becoming clear that while there is culpability to spread around, most of it can be placed on a 24 year-old, inexperienced, reckless, and seemingly incompetent armourer who had already brought live ammo to a previous shoot without following protocols and proper channels.
Still, one has to wonder how she got the job to begin with. The buck always stops at the top. If my first AC can't keep the shot in focus, it's my fault for hiring the wrong AC.
It does appear to be a regular problem on independent films that they'll hire people with less experience to save money and leave them with little support. There's also likely to be a problem with diversity quotas.
The thing is, this is were mostly people on the right are immediately jumping in and insisting that Hollywood needs to change the safety procedures. The more evidence that emerges the more it looks like a dozen safety procedures were already violated.
