addition of patient name and blood type features

Hi @ekayanti, thank you for your contribution.

MedicLog logs key medical information, you can send your medical information to your doctor or you could check your past entries so you can see how your trend is. (History is not yet implemented).
At the moment, the app is only for one user. You said that This application is widely used by doctors and nurses in hospitals and clinics, I don't know where you saw that but I couldn't verified it. For that use, the app needs multiuser capability which was suggested by the PO but not yet implemented.

For that reasons, patient's name does not make much sense since only one user uses the app and you could add your name when you send the information to your doctor.

Also, blood group does not make much sense either because this characteristic of the blood never changes and your doctor already has that kind of information.

I recommend you to try the app before submitting a contribution to check which problems the app solves, also try to improve your mockups.

In Components you should Specify the components/features/interfaces/applications of the software your suggestion relates to, you just wrote general information about the app.
