Ishikawa diagram is a diagram used to determine the root causes of a problem. A visualization tool for categorizing to which factor does the problem is mainly focusing.

The design of this diagram is more likely to it's name which is a Fishbone and the bone itself are the branches determining to what factor does the problem really lies on.

You need to categorize the causes of the problem by using the 8M's:

-Machine (equipment, technology)
-Method (process)
-Material (includes raw material, consumables, and information)
-Man (man power, knowledge)
-Measurement
-Mother Nature (Environment)
-Management ( Leadership)
-Maintenance

Scope of Analysis

The scope of this analysis is to determine the root causes of a problem with the help of those 8M's you can easily find the root cause of why does the problem exist.

Results

Since you can now easily see the cause of the problem then you can now Plan for a solution, Do an action, Check if it is effective and Act for a necessary improvements.
