FOR THIS WEEK, HERE ARE THE CASES FOR YOU TO ATTEMPT.

Case 1

A 50-year-old man has been hypertensive for a long time (10 years). He has not been taking his medications as at when due, in fact, most times, he skips it for weeks and even months.

Last Night, he was going to the toilet and suddenly developed a severe headache while straining to pass stool. His condition got worse and worse, at a point, he noticed he couldn’t move the right-hand side of his body anymore.

He was brought to the emergency room this morning when the doctor spoke with him, he could barely respond. It seems he’s in a coma. Family members said his speech was muffled before he was rushed to the hospital and his face was already tilted to one side.

  • What do you think is this man’s medical problem (diagnosis)?
  • If you were a doctor, how would you help him recover?
  • What lessons have you learned from his story?

Case 2

A 67-year-old Man has been coughing for the last 5 months, He has bought several cough medications which did not resolve the cough. He even tried the herbal medication one of his friends recommended which wasn’t helpful.

Last week, he started noticing that he sweats seriously at night and it drenches his bed, he is not comfortable with this anymore so he makes up his mind that he would go to a hospital to lay a complain the next day at the hospital.

He tries his clothes that morning and finds out he really needs to change them because he seemed to have lost weight. The Nurse confirms after checking his weight that he has lost over 10kg .’ what is really going’.. on he pondered as he waits patiently to tell the doctor all his complains including the weight loss discovery.

Tell us about yourself

I am @desmoniac.. Real name "Udekwu Chinedu Emeka", an Igbo man and a biochemist by profession. I joined Steemit in January and was introduced to steemit by @sistem. I am in my mid-twenties, I love reading, writing, playing chess and any sporting activity.


Why do you love @air-clinic?

I love Air-clinic and it is my home because I have found intellectuals and like-minds in Air-clinic... I naturally have a strong drive for anything relating to health and Air-clinic has provided that platform for me.


High and low point on steemit

My highest point so far on steemit can be affiliated to the multiple contests I have won and being a space force member (rank SF5), while my lowest point on Steemit was being a newbie.


How do you balance your steemit life and real life

Combining steemit life with my real life is pretty much easy for me. I do not have a specific time for steemit and so I regard it as any other social network where I spend my leisure time. However, I try my best to make the best use of my leisure time. So if am not working in real life, I am steeming with my leisure time. This makes things pretty easy for me.


Motivation to study your career.

I actually wanted medicine and surgery, but I was given biochemistry and since I started studying the course, I found out that it's actually the backbone of medicine and I have fallen in love with it ever since.


Challenges/survival story in your specialty

As a young/fresh graduate of biochemistry, I haven't started practicing because I am currently undergoing the compulsory National Youth Service Corp. Regarding steemit, I always faced the challenge of getting my post curated when I was a newbie, the terrible gadget I used also made life so difficult then... I wasn't able to join discord and I struggled to promote my posts. I, however, survived through winning contests, because I always gave each contest my best shot. soon I was able to raise money to buy a new gadget and here I am today.


Final words

Medicine is all about saving lives and adding value to humanity, so if the natural urge to add value to humanity isn't there, you can't stress it, I will advise you find your motivation and be your own motivation!... Find what you love doing and don't hesitate to do it no matter what it may cost you.

@cryptopie , this is an august visit from you. I think the doctor would also think same about the two cases. Did you learn anything special from those stories that you would love to share?

I am a dialysis patient and I just happen to learn some of the diseases and their symptoms from reading and experiences. I had been managing my condition for 15 years now plus my leontiasis and bone pains and it has been hard.

sorry about your health challenge, i know its not really easy coping with the sessions, but i am glad you have diverted your mind elsewhere for the past 15 years. you can always talk to me if you have any problems you want to share with a friend. I am happy you are still positive about life judging from your blog activities.

For case 1

My diagnosis is stroke ( left hemispheric CVA, probably heamorrhagic)

Investigations – MRI, CT if more than 48hrs, Clotting profile, RBS

Treatment – nurse as an unconscious patient, maintain blood pressure high enough to maintain brain perfussion, control blood sugar, treatment for cerebral oedema or raised ICP, IVF.

For case 2

My diagnosis is tuberculosis

Investigation – CXR, sputum AFB, GeneXpert, PCV, SEUCr, LFT.

Treatment – Anti Kochs.

Case 2

Dignosis: Presumptive Tuberculosis (due to cough for more than 2 weeks and night sweat)
Patient to go for a GeneXpert test and Sputum AFB
Treatment commences if: either test confirmed d presence of TB Bacilli
Start First line TB drugs if only MTB is detected (but not RIF resistance)
Start Second line drug if either result showed MTB and RIF resistance
Lesson learnt: Medical attention is needed for any unexplained condition
Resorting to self medication is very bad and might lead to complications
called Drug resistance
At 67 years old (already at old age), the immume system is more likely to get
compromised, hence, the need to go for regular medical check up
