2 September 1998, the pilots of Swissair MD-11 saw smoke in the cockpit less than an hour after take-off. They decided to dump the fuel and land as quickly as possible.
Unfortunately, a few minutes later, MD-11 fell into the Atlantic Ocean. The fire was caused by a defect in one of the cables, which melted and then ignited the insulation, which was made of highly flammable thermoplastic material.
