7 September 2011, Yakovlev Yak-42D carrying the professional hockey team HC Lokomotiv crashed during take-off near Yaroslavl, Russia. Of the 45 passengers, only two survived the crash.

However, 5 days after it, one of them - Aleksandr Galimov - also died from significant injuries. The cause of the tragedy was blamed on the pilots because, as the MAK revealed, one of them unknowingly held down the brake pedal.
