Who decides the details on a bitcoin fork?

My belief was that once the bitcoin blockchain was active, they couldnt modify it. im wondering even if this isnt true, who/what/when/ decided the new names for the forks, like bitcoin cash, and bitcoin gold? what information do they get to see that tells them "this" is when the fork is, or how can they suddenly decide that segwit is going to be delayed? or am i wrong in thinking that segwit and any other fork has anything to do with bitcoin besides the original block-chain after the fork?

Anyone can fork but getting community consensus to use your code is tough. At the end of the day it's a marketing job and if too many high profile forks take place it will devalue all bitcoin over time.
