I agree, I just have a comment regarding the number 2. One-way conversation. This is based on my experience, some leaders allow their subordinates to speak BUT they would always decline the suggestion without explanation or the leader is not interested. So in the following meeting, the subordinates won't give any suggestion because they know that the leader will just decline it. So during the meeting everyone is quiet which makes the meeting more boring.
