Well as you said, most of these are not enough to disqualify an RPG alone, but when in combination you're right, they're usually quite bad signs. I find that I mostly agreed on things like dialogues, progression and world depth, in all honesty.

The loot system is something a bit different, because it ultimately boils down to personal taste. I, for one, like loot systems that are 1) random 2) full of junk items. Am I a masochist? Probably, but I like to work for good loot, and I find that common items help immersion. I actually modded my copy of Oblivion with a bunch of stuff such as Armamentarium to have hundreds of different items that added absolutely nothing in terms of value, I just liked the idea of a world where no two weapons\armor\objects were the same. But I never added mods that said "I created a sword that's really cool and I've placed it in that chest in particular", because I don't want to own the sword, I want to find the sword out of sheer luck and effort. But again, that's me. My secret perversion would be to keep selling junk\common weapons and armor to vendors and eventually see those items spread among guards, citizens and townspeople.

Then again, I'm replaying Arx Fatalis and I realize that most of these things depend a lot on the kind of RPG that we're talking about. Story-based, closed world (is there such a thing, as opposed to "open world"?) RPGs usually work better if everything is pre-placed and pre-calculated.
