What is carpal tunnel syndrome? Write it down in your own words after gaining knowledge from the lesson post.

After studying this first week's lesson, I can conclude that carpal tunnel syndrome is a condition that may occur due to several causes, but the main thing that makes this condition occur is repetitive use, not only that but there will be nerve compression around our wrists. We need to know that this condition is not a disease that is inherited by heredity or genetics.

  • Causes.
    The causes of carpal tunnel syndrome usually involve several factors including: Median Nerve Compression, Repeated Pressure or Overuse, Mismatch of Nerve Size and Carpal Tunnel, and Hormonal Changes. In this Median Nerve Compression, the median nerve that runs from the forearm to the hand by passing through a narrow pathway called the carpal tunnel is often even compressed, thus becoming the main cause or trigger. As for Repetitive Pressure or Overuse Activities such as typing on a laptop / PC for a long time and continuously or it could also be by using a vibrating device that is likely to damage the nerves and become weak muscles in the wrist area. While in the Mismatch of Nerve Size and Carpal Tunnel this occurs when the median nerve is larger than the capacity of the carpal tunnel, and nerve pressure is usually inevitable. As for the cause due to Hormonal Changes, it occurs because hormonal fluctuations during pregnancy or menopause can also affect this condition.

  • Symptoms
    For carpal tunnel syndrome patients, it is common to show major signs including: Disturbing pain, numbness and tingling, and muscle weakness.
    As for when the annoying pain occurs, it will certainly feel pain around the wrist, hand, and fingers, often worsening at night. Meanwhile, when symptoms such as numbness and tingling occur, they will certainly be felt in the thumb, index finger, and middle finger due to nerve pressure. Meanwhile, when symptoms of muscle weakness occur, repeated use can make the forearm muscles lose their strength.

  • Types
    There are several types of carpal tunnel syndrome, including Acute CTS, Chronic CTS, Primary CTS, and Secondary CTS. In the Acute CTS type, symptoms will usually appear suddenly, generally occur due to trauma or even due to injury, and last in a matter of days to weeks. In the type of Chronic CTS it will be characterized by the presence of symptoms that develop slowly which are usually caused by repetitive activities so that they can last for weeks and even months. Whereas in the Primary CTS type, the cause is not yet known. As for the Secondary CTS type, it is triggered due to other medical conditions such as pregnancy, or diabetes, thyroid, or trauma or the like.

How is carpal tunnel syndrome diagnosed? Is there a clinical examination or assessment test?

After I have read and studied the material provided well, I can conclude that Carpal Tunnel Syndrome (CTS) is diagnosed using 2 basic methods, namely the General Method and Special Tests in Physiotherapy. Well in this general method will include a Comprehensive Physical Examination where the doctor or physical therapist here will evaluate the condition of the patient's wrist, then the hand, and also the fingers with the intention is to find out signs of pain or numbness or weakness. Electromyography (EMG) is then performed which is used to look at the electrical activity in the muscles and is helpful in determining possible nerve damage. Finally, a Nerve Conductivity Study (NCS) is performed which will look at the speed and strength of electrical signals that occur in our nerves to determine the presence of median nerve compression.

Friends all on the Special Test method in Physiotherapy which is used to strengthen the diagnosis, there are many special tests commonly used including: Phalen Test, Film Test, Reverse Phalen Test, Tinel's Sign and Median Nerve Compression. In the Phalen Test it will usually involve flexion of the wrist with a duration of about 60 seconds in order to know symptoms such as numbness or pain. While the Film Test is utilized by pressing the wrist in order to see the response to the median nerve. Now in the Reverse Phalen Test which is almost similar to the Phalen Test, but there is a slight difference, namely in this test it will utilize the hand by putting it in an extension position. The next is the Tinel Mark test, which is by tapping the wrist area in order to stimulate a tingling sensation or trigger pain along the median nerve. And the last is the Median Nerve Compression test, which in this test will be done by pressing the median nerve directly in order to see reactions such as pain or tingling.

Arm Prayer Stretch

The first one I chose is the arm stretch during prayer, How to do this stretch is easy, namely by patting both left / right hands and facing up like the position of a praying person and placing it in front of the can first stretch from top to straight down right in front of the chest. Certainly by doing this type of stretching will help us to get rid of the unpleasant feeling or stiffness that is a little pain in the arms and wrists.

Stretches for CTS

This exercise is done by grabbing the hand with a straight arm, then the wrist that is grabbed will be bent down slowly. And do it over and over again.

Median Nerve Glides

This exercise is done in a very easy way by placing our arms on the wall with our fingers pointing down, then our heads will be turned left and right and done repeatedly.

As we should know, arm stretching exercises for prayer, stretching for CTS, and median nerve gliding exercises are very effective in keeping our hands and nerves healthy. After doing the arm stretches, I certainly felt relief from muscle tension and increased flexibility, perfect for relaxation after strenuous activity.

Whereas after stretching for CTS, I feel lightness in the median nerve and I also feel a reduction in the feeling of kabas, it all works when I do it regularly.

Meanwhile, after doing the median nerve gliding exercise, although it feels a little strange at first, but after repeating it a few times, it will feel good and relieved, the stiffness is gone. So from the three exercises that I have practiced are simple but very useful for the prevention and recovery of CTS.
