Dr. Kenzhou Tenma, an elite neurosurgeon recently involved in a relationship with the daughter of the hospital director. One night, a seemingly small incident changed Dr. Tenmar's life forever. While preparing for surgery on someone, he was forced to switch patients after receiving a call from the hospital director and had his brain operated on to save the life of a famous actor. His colleague, fianc, and hospital director praised his achievement; But due to the switch, a poor migrant worker dies, causing a crisis in Dr. Tenmar's conscience.
So, when a similar situation arose again, Dr. Tenma opted for surgery on the youngest son, Johann Libert, instead of the mayor of the city. Unfortunately, this choice led Dr. Tenma to a deadly mess - his social status being one of them. His girlfriend leaves him, everyone in the hospital starts talking about him, everyone including the director is dissatisfied with him.
However, the mysterious death of the director and two other doctors restored Tenma's position. He was released without any evidence to prove his guilt. After the incident, the boy who survived due to his treatment mysteriously disappeared from the hospital. Meanwhile, when a detective is given the responsibility of the case, he begins to suspect the doctor. But he can't do anything because there is no evidence.
After the director died, the new director came and said. Tenma regains his old position. Everything was going well, but then came the real twist of the incident… suddenly another murder took place and a thief who witnessed the murder was severely injured and lost his memory.
