"Ladies first" is a sexist term, usually derived from the idea of "gentlemen" who are supposed to treat women in a certain way. Gentlemen's behavior towards women is utterly sexist and misogynistic.
This behavior arrives from the opinion that women are somewhat weak mentally and physically. That they are lower in a hierarchy so they have to be patronized with such behaviour.

On a small scale by oblivious sexists, yes. On a large scale as a society, no.
But we have to remember that a lot of women expect to be treated in a sexist ways as they are brainwashed by a sexist culture that it is the behavior that they should expect from men.
