The morning task for the day started smoothly and exciting. I arrived at the hospital at exactly 8:00am and we were to begin the pharmacy ward round at exactly 8:30am, guess I wasn't so late after all.
First my colleagues and I waited for our preceptors to lead us to the wards. They arrived on time and without further ado we moved to the postnatal ward where women who just delivered their baby stayed.

We were assigned patients, each person had a case to follow up. I picked the folder which I was assigned, read through to understand the condition of my patient. Then move further to obtain subjective data from the horse's mouth, that's the patient herself.
Upon interaction with my patient, I realized she was allergic to paracetamol (acetaminophen) as common as it is. It was causing her swollen legs and face, even making it difficult to breathe.
Obviously, the role of Pharmacists in the ward is to establish pharmaceutical care to patients, Identify drug therapy problems, ensure strict compliance to patients via patient counseling amongst other professional duties.
So to render pharmaceutical care to this patient and establish interprofessional relationship, attention of the physician was drawn and a new prescription was immediately issued for a change in medication.
I hope to ensure close pharmaceutical care monitoring of this patient until she is discharged.
After the interesting ward round, we moved to a location where we will discuss what we saw in the ward , possible outcomes and prognosis as well as alternative treatment approaches.

This was a beautiful platform for learning and conversing with intelligent minds. Ideas were established, reference books were referred to until we got a conclusion.
On our way back to our various units, we decided to take some photos, probably also for the sake of memories.
We returned to the units to carry out our unit activities including, costing, dispensing and documentation of patients' prescriptions.
Gradually, we kept working till it was the officially closing time and all workers departed to their various destinations.
It was over for me yet, I still have to resume at the community pharmacy outlet.
Not so stressful, work went smoothly and there were no complaints whatsoever. Clients were satisfied with the services rendered and we all left the pharmacy happy.
