What is my medical problem? Ok, here it is!
Diet: If heart failure is present, you should lower your daily intake of sodium to 2,000 mg or 2 g or less per day, eat foods high in fiber and potassium, limit total daily calories to lose weight if necessary, and limit intake of foods that contain refined sugar, saturated fats, and cholesterol.
What my doctor tells me all the time, but I love all the things bad for you. Last visit, gained more weight as well as stressed that increase of another medication full fills my daily in take of pills daily to 11 daily. That is seven days a week, 365 days a year.
Monitoring your weight: This involves daily recording of weight, increasing your activity level (as recommended by your doctor), resting between activities more often, and planning your activities.
Which I don't do, don't ever weight myself till I go to the doctor. Never found a weight scale that will last let alone one that goes over two hundred pounds that I can afford.
Avoiding tobacco products and alcohol.
I have given up both of these and even went as far as to give up what I drank since a kid Pepsi. Doctor's daughter owns stock in Pepsi and how I learned that is we discussed I was first that tried to say hey I gave up smoking, as well as drinking. I should keep my Pepsi. Nope, diet if I must. I don't like diet, as tried it. Then find anything that takes place of cola, but stop now was doctors demand. Finally discovered Coke Zero, still tastes like the real sugar thing. Drink maybe 64 oz. daily. No, never have drank water unless in foods cooked. Worked where how water is cleaned, and even if coke is made, it is in my head that is different. Why I drink so much Coke Zero now, but doctor leaves me along on that taste now for last few years.
Regular medical checkups: During follow-up visits, your doctor will make sure you are staying healthy and that your heart disease is not getting worse. Last visit, was gained a few pounds compared to last visit, so wasn't happy, but try to lose and not gain. Hard to do when I won't or can't walk without getting tired. Use or suppose to use a breathing cpap machine. Told him if really hard to breathe then I will use it. Stated one day I will not wake up then I will be sorry. Said, no I'll be dead, so won't care. Not funny, maybe but at 70 years old and majority of friends have already went to earth so what is holding me back?Doctor and all these pills.
Also have a blood pressure machine and don't use it unless requested which he doesn't do any longer except if I feel light headed or take a fall and break something. Pill's have prevented a lot of that so far. Still have the broken shoulder and two broken fingers that I didn't allow to set correctly, but work fine for me. Yea, one of those that if I don't want it on me then it's not going to stay on me. splints, casts and so on. Now the medical word that is used is called Hypertensive Heart Disease , with high blood pressure. There that should make everyone happy. In short I get tired just taking a shower. or walking to the living room from my bedroom. Now, that is bad. I have not reached the riding chairs yet, did use a cane for awhile but felt too old so reason for the falls and still have cane in truck of car should I decide it's time. Now, for some jelly beans, I love those things especially the jelly bellies.

There's no evidence linking high sodium intake to heart disease. In fact, there's more evidence suggesting that sodium intake that is too low could be far more problematic to your health. Doctors continue to parrot this myth with no scientific foundation (I have learned that doctors are not necessarily people of science).
Excellent willpower for you to give up both smoking and alcohol. I don't think I could ever give up beer, but I have nothing and no one to live for.
Weight loss is absolutely critical for your health to improve. If you lose weight then all the other measurements will improve, exercise will be easier, and your stamina will improve dramatically. Diet is far more important for weight loss than exercise. 90% of weight loss is dietary. Calorie restriction is the only way. You absolutely must lose weight. That is your #1 goal.
64oz of soda per day!? Even with sugarless soda, the acidity is going to melt your teeth away. There are no health concerns surrounding artificial sweeteners, so no worries there. They are the most studied food additives in history. Still, that's a hell of a lot of soda, but if you need it you need it...
Give up the jelly beans, it's pure sugar. Try shifting to a more plant based diet with more fiber. That will not only help you lose weight, but is positively correlated with improvements in heart disease and atherosclerosis. Even "lean meat" is around 50% fat, and it takes only 3% fat calories to store fat as fat. With complex carbohydrates, liponeogenesis (the creation of new fat from non-fat sources) has an associated calorie cost of ~30%. You are much better off eating potatoes, rice, carrots, etc, than you are eating "lean chicken breast." And throw some salt on there for flavor.
Study the "salt problem" and bring your doctor data.
Good luck.
