I work in the movie trailer industry. Mostly the hierarchy, size, and placement of names is dealt with within the actors/actresses contract negotiations. So usually the contract is defined long before production on the movie starts and long before any of the design of the promotional materials has even been thought about. It drives us designers crazy too, so we feel your pain. Basically it amounts to... lawyers.
