Planter fasciatis


If I talk about planter means palmar surface of foot and fasciatis is composed of two words.One is fascia which is the tough structure surrounding muscles and itis in medical meaning inflammation. So this is a condition of inflammation of planter fascia. Now the question arise that what are somethings that cause this condition so below I am talking about some causes;

Causes

• If someone is bulky then body weight will increase and you know that our foot bears whole body weight that's why it can cause inflammation in foot Palnter side so fascia become inflamed.
• If arch is absent in feet naturally then flat feet can cause inflammation of planter fascia.
• If there is high arch presence in some people then it can also cause abnormal transfer of body load in foot and hence cause inflammation.
• If there is any thing wrong in body poster then it can also cause inflammation in planter side of foot because of abnormal transfer of body weight.
• If someone have abnormal pattern of gait then it can cause fascia to swell.
• If someone have prolonged hours of standing like if someone have a job of this kind then due to prolonged standing this issue may cause.
• If someone wears heels in normal routine then it can trigger or provoke this condition.

Signs and symptoms

• If there will be a patient suffering from this condition then he will complain about severe pain and tenderness upon touching at planter side of foot.
• If there will be patient suffering from this condition then he will appear with redness and raised temperature of that area in most cases.
• If there will be a patient suffering from this condition then visible swelling will feel around heel and under the foot.
• If there will be a patient suffering from this condition then he will complain that he cannot walk for more than 5 minutes.
• If there will be a patient suffering from this condition then he will complain that doing any physical activity that involve foot increase the pain and rest relieve the pain.

Treatment

• For reducing pain and swelling patient always need to use painkiller and anti inflammatory medicine at least one time for a day.
• For reducing pain and swelling icing is most effective so it is advisable for patients to apply cold pack at their swelled area for three times in day,with 10 times each repetition.
• Exercises are always important to subside edema or swelling that seems to be very apparent so for this heel raise, rolling,calf stretch,toe curls etc.
• With a continuous treatment of 2 months patient will feel significant effectiveness and after 6 to 8 months condition will heal properly


Diagnosis of planter fasciatis


If I talk about planter fasciatis then it can be diagnose in following way;

• If someone is complaining about planter fasciatis then doctor should ask about symptoms of redness, swelling, pain and tenderness if these are present so if these are present then it means that now it's important to know location of all these symptoms so if these are under foot and around heel then this is most probable that these are symptoms of planter fasciatis.
• In next step doctor should ask patient to move foot up and down so if range of motion of foot is limited in both movements then it means that there is some restriction and it can be swelling and pain which is not allowing patient foot to move through complete range of motion.
• In next step doctor will ask patient to walk for some steps to analyse gait of patient because if there is any abnormality in biomechanics then it should be corrected first because that abnormality can lead to this condition
• In next step,talar tilt test and pronation test will be performed which are useful for assessing range of motion of ankle joint and for assessing foot rolling inward respectively so if these are positive then it means that 80% of diagnosis is confirmed and patient is really suffering from planter fasciatis.
• At last definitely imaging investigation tests are important so for checking any soft tissue injury we will recommend patient to go through medical resonance imaging of foot to check that is there any soft tissue including ligaments damaged or not or more deeper study ultrasound can also be used.

This is I am curling my toes of foot with hold of 10 seconds and then giving them 5 second relax with 10 minimum repetitions

This is I am calf stretch with hold of 10 seconds and then giving them 3 second relax with 10 minimum repetitions.While performing I am turning my foot to show that how much stretch is needed normally but it's intensity varies from person to person according to muscular tension

This is I am performing heel raise with hold of 10 seconds and then giving them 3 second relax with 10 minimum repetitions


Exercises reviews


If I talk about giving reviewing of all the exercises I performed then all were effective for me so let's talk about them one by one;

• I performed toe curls in which I curl my foot toes and hold them and while doing this I was feeling little pressure at ball of my foot because I did this exercise first time so in start I was not much comfortable with 10 seconds hold but after that I felt good when I completed 6 repetitions.

• When I performed calf stretch then in start I was feeling that I am stretching something which is elastic and there is not much elasticity in it because I was feeling prominent stiffness in my these muscles so after five stretches I was feeling that as that muscles are getting soft and going back for actual elasticity which they should have so it was most effective.

• When I perform heel raise then it was enjoyable for me because I also perform it in my routine specially during my morning walk. One plus point that I noticed is that holding for more time gives more strength to muscles because in my routine I hardly hold it for 3 to 4 seconds each repetition but for participation in this course, I hold it for 10 seconds then it was effective for five repetitions even so this is I learnt from this exercise.

In the second question, you tell us about how you have to assess a patient by doing physical examination and history taking, performing investigations and specialized tests. But you didn't add the imaging tests name. It would be better if you add them too. Great.

Task 3 (3.3/4)
You try the toe curls, calf stretch and heel raise. You did the toe curls and heel raise correctly. But in calf stretch, you can't stretch calf muscles in such position, secondly make video from wide angle for better understanding And at last keep your leg straight then perform calf stretch. Always remember to cold therapy & rest is the best therapy in the treatment of plantar fasciitis. I appreciate your efforts.
